Any Coach, Player or Parent requesting a guest player, a dual rostered player, permission to travel out of state or requesting to transfer must print and fill out the proper form.  Form must be presented to the FSA office with proper signatures.  The request will be fullfilled upon authorization from the FSA Director of Coaching and upon payment of fees involved.
